There are many reasons why your Site is compromised or may be compromised.
A Malware/virus/spyware on your computer might steal your FTP password where the hackers use the password to login to your site root > Change your password after every preset period for safety. This includes ftp, site, email passwords
File Permissions are a very important aspect. Most code injections are being done to Index files, either at the site's root or the site's Templates. Change the file permissions to 444 before your site falls victim. For Joomla users cchange your index.php and template's index.php file too
Unused compressed folders > never keep compressed folders on your server. Mostly webmasters forget to delete a site they uploaded on the server as a zipped folder
Idle Templates: If you have templates on your site and you do not use them, please delete them. they might be violated by hackers
Installed Add ons/ modules / plugins: Can you trust all the add ons/plugins etc you have installed? Most have vulnerabilities that you do not have an idea about, but your conventional hacker knows. Keep your addon scripts up to date and uninstall/disable ones you never use
